The applicants who lost their passports are required to call over the Consular Division of the Embassy personally and face an interview and hand over the applications.

Required Documents

  • Duly filled Form K-35A

  • 4 colour photographs [3.5 x 4.5 cm] taken within 3 months before the submission (both ears and forehead should visible, white colour background and dark colour dress code)

  • Original Birth Certificate obtained within 06 months

  • Sri Lankan National Identity Card (If available only)

  • Proof of Applicant’s Visa Status – SOGGIORNO

  • Original Police Report issued by the Police Station (Questura) to which complaint was made regarding the loss of the Sri Lankan passport (Passport Number of the lost Passport should be included in the Police Report).

  • Duly filled declaration form ‘D’ (to be filled by the applicant who lost the passport)

  • Profession

    • If the Profession needs high level of qualifications, (ex: Doctor, Engineer, Accountant, Quantity Surveyor, etc.,) the application must provide educational qualification or professional membership status.
    • For other executive professions, a service letter should be provided.
    • For unskilled jobs, (Ex: labor, housemaid, no document is required).
    • If the profession is already included in the previous travel document, no document is required.

  • Relevant fee

    EUR 330 – If copy of the lost passport is available

    EUR 420 – If copy of the lost passport is not available
