Deaths of a Sri Lankan national can be registered and death certificates could be issued at the Embassy provided the following conditions are satisfied:

The deceased be a citizen of Sri Lanka, and the death had occurred in Italy, Greece, Cyprus, San Marino, Albania and Malta. An application for the registration of a death may be made by the nearest relative of the deceased who was present at the time of death, in attendance during the last illness of the deceased or who took action to dispose of the body of the deceased by burial, cremation on by other means.

4.1.Declartion of Death under Section 30 Registration B 11 to be submitted (if the Application for registration of the death is made within three months of the death)

4.2. Declaration of particular relating to the death for purposes of late registration (Section 36) – Form – Registration B 15 to be submitted (if the Application for registration of the death is made after three months of the death).

Required Documents

The original documents should be submitted with two photocopies
  • The death certificate issued by the Competent Authority of the country in which the death occurred

  • The Post-mortem certificate issued by the Competent Authority of the country in which the death occurred

  • Passport of the deceased valid at the time of death. If the deceased is a minor, the passport of the parent in which the endorsement regarding the deceased is made

  • Original birth certificates of the deceased

  • If the human remains have been repatriated to Sri Lanka for burial/cremation, the original of the Airway Bill ( Air Cargo Bill). If the human remains have been buried/cremated in Italy, the burial permit or a report issued by the concerned authorities on the disposal of human remains

  • The original passport of the person who submitted the documents for registration of the death

  • If the application for registration of death was made after one year a letter explaining the reasons for such delay must be attached

All application made after one year, will be forwarded to the Registrar General’s Department, Central Record Room, Maligawatta, Colombo 10, Sri Lanka, Tel : 00-94- 11 23 29 773,Fax : 00-94 -11 24 33 075 for an approval. In such cases the death certificate will be issued only after approval is received from the Registrar General’s Department.

Obtaining a fresh copy of birth, marriage or death registration registered at the Embassy of Sri Lanka in Rome

  • Duly completed application form

  • Passport of applicant, supported by marriage, birth certificate to establish direct connection to the those listed on the Certificate. ie: if a fresh copy of a child’s birth certificate is being requested, the parents certificate of marriage along with the Passports

  • Relevant fee in euro. Fee: Euro 30/- subject to change without prior notice
